Sildenafil, sold under the brand name Viagra, is the most sought-after erectile dysfunction drug in Korea, but it also has a reputation for being a performance enhancer, especially when taken on an empty stomach.
For this reason, doctors prescribe sildenafil as the first-choice drug for erectile dysfunction, particularly if it has been in the market for more than a decade. Sildenafil, the active ingredient in Viagra, is also approved by the FDA to treat pulmonary arterial hypertension.
For the first time, Korean researchers have found that sildenafil boosts blood flow to the penis, which may help to increase blood pressure, but this is only temporary. The study, published in the journal,Pharmacotherapy, is an open-label, randomized, double-blind, placebo-controlled clinical trial that compared sildenafil with placebo for improving erectile function in men with erectile dysfunction. The study found that men taking sildenafil had an average improvement of 5.1 points in their erectile function compared with those taking a placebo.
In this study, participants were recruited between April 2009 and May 2011 from 13 clinical centers in Korea. They were randomly assigned to receive sildenafil or placebo for a total of six months and were then followed up for six months to determine if improvements were observed. Sildenafil, or the generic Viagra, was compared to placebo for six months.
In the study, participants were evaluated for their ability to achieve and maintain an erection during sexual activity. The main endpoints were overall erectile function, which was measured using the International Index of Erectile Function (IIEF).
For the men in the sildenafil group, the mean change from baseline in IIEF was −2.9 points in the placebo group versus −1.1 points in the sildenafil group.
The mean change from baseline in the IIEF of the men in the sildenafil group versus the placebo was 1.8 points in the sildenafil group versus 3.8 points in the placebo group, which was statistically significant at p=0.003.
The sildenafil group had a similar mean increase in IIEF of 4.9 points in the placebo group versus 1.6 points in the sildenafil group.
However, a higher number of participants in the sildenafil group was associated with a lower overall sexual desire, and a higher percentage of participants in the placebo group reported an orgasmic dysfunction.
Sildenafil had the lowest number of participants achieving an erection, with a median of 3 participants in the sildenafil group compared with 2 in the placebo group.
Overall, sildenafil was well tolerated, with side effects such as headache, flushing, nasal congestion, and back pain that were reported in the trials.
However, in patients who took the drug for less than two weeks, the sildenafil group reported a significant improvement in their erectile function, with improvements in all the measures measured. The sildenafil group also had a higher percentage of patients who reported a higher desire for intercourse compared with the placebo group.
It’s possible that sildenafil may have been more effective in improving erectile function than Viagra, but the benefits on sexual performance are not entirely clear. One study by researchers at Seoul National University found that sildenafil improved patients with erectile dysfunction by more than 25 percent.
